When the guard says to the patient that they have known eachother for centuries, I interpret it as two hemispheres of the brain talking to one another, The guard represents the Left Hemisphere, on control of the right side of the body this hemisphere is what is conscious, it is where we dwell, it is what understands language. The patient represents the Right Hemisphere of the brain which is the creative side, it comprehends music, art, and spaces.
