Not sure.
I am not a game developer, but I knoow that 1 bad decision can completelly de-rail an entire project.
Fallout 4 for instance was killed the moment someone thought they might include a dubbed protagonist. This simple decisiom multiplied price of every single branching decision. And because of that, you can no longer roleplay.
I still have no idea why they thought the most anoying faction should be the ultimate faction that cannot be destroyed. Instead of Yesman, who was invincible in New Vegas, they made invincible the FIRST NPC you meet and is anoying, so people got a bad impression of the rest of the game.
And last... Oh god, why is the scale so small.
I still remember only 2 cities in the entire game. 2 cities.
If there are more, they are criminally well hidden, But with 2 + 3 faction homes, that makes about.... 5 places on the entire map that will matter to you.
I have no idea what kind of development hell was F4 in, but it's hard to immagine it was made by the same people who made Skyrim.
