@LukeAlmighty @kaosktrl this drug damages the cells that act as a 1-cell-thick & semi-permeable barrier between your blood vessels and the rest of your body in such a way that delays their ability to reproduce and thus heal injuries or maintain their population.
This causes blood to seep out into the tissue where it eventually dies and bacterial consume it. Gradually this leads to the decay of tissue, especially at the immediate injection site. Heroine, Fentanyl and other drugs do this also, but this particular drug, which is a common Fentanyl & heroin addition as it extends the high, is especially effective at damaging the endothelial cells.

@LukeAlmighty @kaosktrl I'll add that it's not these festering wounds that are actually killing abusers of the drug; The overdose-reversal drug naloxone is used to prevent a death by overdose in the event of an **opiate** overdose, but this particular drug is not an opiate. It's partially effective- but only partially. The breathing disruption produced by Xylazine isn't counteracted, meaning that even with treatment you're still liable to suffocate if you take too much.
The key advice is, as always, the same; winners don't do drugs.

There are two routes that are pernicious though.
Social drug abuse: your friends are into something light, gradually get into heavier shit, and drag you into hell with them.
Doctor fucky wucky: You injured yourself, you were prescribed an opiate, you got addicted to the opiate your doctor prescribed for your pain, your prescription ran out, as a result of withdrawels your experiencing a lot of bodily pain, so you seek a replacement opiate under the assumption your injury still hurts, or just to stop the pain.
