Overclocked? If no, look for a replacement.
Also, check if your display cable is ok. I've mistook a bad cable for gpu artifacts before.

Depends on what exactly you saw. It's not impossible, but unlikely unless you have multiple stuff that used the GPU at the same time. Like your DE, a game and a video playing via hardware decoding in the background.

If it's a whole screen glitch, it's likely a software bug or cable problem. If it's just the texture on a 3D model, or part of a model, that's usually the telltale sign of GPU misfiring.
