One of the scariest moments I've ever had in a game was while playing Minecraft. It was like 2 or 3 in the morning and I was doing some spelunking in a cave system when I hear an Enderman start talking. I had always played with friends before then so we were always talking over the game, and I didn't realize that Endermen could even talk. So I heard this distorted "what's up?" come from the game and it sent the worst chill up my spine because I knew I was alone but I was hearing something talking to me.

The scariest one was while playing PT, I'll post about that one when I'm at work or something because it genuinely freaks me out still to think about it.

So I had played PT several times, gotten the hidden ending, seen all the glitch effects, etc. My friend and I were showing it to another one of our friends who was into horror stuff but hadn't played it yet. I can't remember the exact words, but a voice in the game said something along the lines of "stop" while we were laughing at something else in the game and it immediately silenced us because we hadn't heard that before. Our other friend was like "what's so scary about that?" and we had to explain that it hadn't happened before. I was borderline crying I got so spooked by it. I have not been able to find anything online about that occurrence but I have 2 witnesses who heard the exact same thing. Maybe a glitch? Maybe the microphone picked up what it thought was laughing and triggered an event, or the certain way we were moving in game? I have no idea but I know that it happened and the fact that I can't find anyone else who experienced it besides us 3 is really freaky.
