From watching Trash Taste, Japan has a lot of retarded government bloat where getting even the most basic paper work done is task for a day with a ton of fetch quests. They also ask for your name, address and phone number everywhere so I assume they have pretty retarded KYC laws.
I like that people are more considerate so smokers don't blow smoke in your face and sick people don't caugh in your face, but that comes with a downside of people still wearing masks everywhere even though there isn't a reason to. However because of such social pressures plus maybe a bit zealous laws and possibly genetics, their public heathcare isn't as expensive as it is here.
