AI CSAM is regarded as such because it is trained on actual children and in some cases, actual child porn. it can very easily be abused by taking photos of actual children and using AI to undress them and there are minors who are actually dealing with this sort of trouble in school.
even a place like allthefallen has extremely strict rules on how realistic AI can get. baraag bans realistic AI images. there's a reason for that.

I've never, ever heard of what you describe, and I made a post years ago about how people on Pixiv were doing actual traces of RL CP. That was not AI-generated, that was those people doing that themselves. THAT stuff was actually realistic because it was literally TRACED BY HAND.
Funnily enough, in that post I joked about the BBC just discovering the "pedo" tag but ranting about AI art (which back then was shit anyways, there was nothing "realistic" about it). The pedo tag itself encompasses all the "lolicon" (3D and non-3D) on Pixiv anyways (surprise surprise, Japanese at least are honest). But that's all an aside.

stable diffusion is the program used to generate stuff, models are what contain the trained data.
