It's retarded.
They are the same. Sound the same etc. But, you write ú at the begining of the word, and ů everywhere else.

With the exception of connected words. (troj) + (úhel) = trojúhelník.

But that's it. It's the dumbest, yet simplest rule in Czech language.

Oh no... You have to remember 2 extra symbols next to the usual alphabet.

Well, 3. But ů is literally ú, so who cares.
